Our Home Care team works closely with and under the supervision of your own physician.
WHO IS ELIGIBLE?
A person qualifies for Home Care if:
·A physician determines Home Care is needed and the home environment is safe and appropriate.
·Their condition requires intermittent nursing, physical therapy or speech therapy.
·It is difficult to leave the home without mechanical or personal assistance or leaving the home is medically inadvisable. (Many private insurance companies do not require homebound status.)
Heartland Home Care provides services to all eligible patients without regard to race, color, age, marital status, sex, disability, national origin or religious practice.
WHERE ARE THE SERVICES PROVIDED?
Our services are provided in the comfort and security of your own home. We can also provide care if you live in an assisted living apartment or even if you are staying with family or friends.
WHY HOME CARE?
In our country’s formative years, there were few hospitals. Health care was provided in patients’ homes as physicians and nurses traveled across this country’s heartland. Times changed and the focus of health services revolved around the hospital and advancing technology. Times have changed again. It has been realized that the hospital isn’t always the best place for many patients - their needs are changing. Patients staying fewer days in the hospital have more educational and care needs after discharge. Terminal patients are living their last days in the familiarity and love of their home. Health care is returning home, but this time a full home care team along with current technology is coming with it. All of your health care needs under one roof - your roof!
WHEN ARE SERVICES AVAILABLE?
As a Home Care patient, you have round-the-clock access to professional care. Our office is open and all services are available to provide scheduled visits, 8:00 a.m. to 4:30 p.m., Monday through Friday. Home Care staff are on-call and available to assist patient’s additional needs during evenings and weekends.
HOW DO I PAY FOR HOME CARE?
Most third party payers including Medicare and Medicaid cover home Care costs. Our qualified staff can research coverage issues you may have and will provide billing services for our care.

Heartland Home Care is a not-for-profit department of Hillsboro Area Hospital. Heartland Home Care welcomes donations or memorials that allow us to purchase specialized equipment and expand services to better meet the community’s health care needs.
